Informed Consent & Acknowledgement
Role and Responsibilities: a. The coach/assistant coach/volunteer agrees to fulfill the assigned role in accordance with the guidelines and objectives set by the organization/league. b. The coach/assistant coach/volunteer agrees to attend practices, games, camps, and other team and program activities regularly and punctually unless there are extenuating circumstances communicated in advance.
Code of Conduct: a. The coach/assistant coach/volunteer agrees to adhere to a high standard of professionalism, ethics, and sportsmanship. b. The coach/assistant coach/volunteer will treat all players, parents/guardians, officials, and other coaches with respect and fairness. c. The coach/assistant coach/volunteer will not engage in any form of verbal, physical, or emotional abuse toward players or others involved in the program.
Safety and Health: a. The coach/assistant coach/volunteer will prioritize the safety and well-being of the players at all times. b. The coach/assistant coach/volunteer will follow established safety protocols, including providing necessary equipment and ensuring proper warm-up routines. c. The coach/assistant coach/volunteer will promptly report any injuries or incidents to appropriate personnel and provide necessary assistance as required.
Confidentiality: a. The coach/assistant coach/volunteer will maintain the confidentiality of any personal information or sensitive matters disclosed by players, parents/guardians, or other coaches. b. The coach/assistant coach/volunteer will not share or disclose such information to any unauthorized parties without consent.
Communication: a. The coach/assistant coach/volunteer will maintain effective and professional communication with players, parents/guardians, and other coaches. b. The coach/assistant coach/volunteer will respond to messages and inquiries in a timely manner and use appropriate communication channels as designated by the organization/league.
Compliance with Policies and Regulations: a. The coach/assistant coach/volunteer will comply with all applicable organizational/league policies, rules, and regulations. b. The coach/assistant coach/volunteer will complete any necessary background checks, certifications, or training required by the organization/league.
Coach Availability: a. Our coaching services are subject to the availability of our coaches/assistant coaches/volunteers. While we strive to provide coaching sessions as per the agreed schedule, there may be instances where the availability of a particular coach is limited or subject to change. b. Program or camp staff has a responsibility to make reasonable efforts to inform the organization (westkhoops) in advance if their availability changes and to provide alternative options or arrange for an approved staff member replacement to cover their absence. c. The organization will make a reasonable effort to reschedule or assign a different coach in case a coach becomes unavailable due to unforeseen circumstances or any other reasons beyond our control. d. We will work with the client or customer to find a suitable solution, which may include the assignment of a different coach or a prorated refund of any fees paid for the affected coaching sessions.
Camp Enrollment: a. The participation or involvement of the coach/assistant coach/volunteer in camp sessions is dependent on meeting the minimum enrollment requirements. If the minimum number of participants is not met, we (westkhoops) reserve the right to make adjustments, which may include rescheduling the camp, assigning a different coach, or cancelling the camp altogether. b. If a camp session is cancelled due to insufficient enrollment, facility cancellation, or acts outside of the organization's control, we will notify you as soon as possible. We will make every effort to provide you with the option to transfer participation to another available camp date. c. Our camps have limited enrollment capacity to ensure a high-quality experience for all participants. d. We reserve the right to limit the number of participants in each camp to maintain an optimal learning environment.
Termination: a. The organization/league reserves the right to terminate the services of the coach/assistant coach/volunteer at any time for reasons including, but not limited to, violation of the code of conduct, failure to fulfill responsibilities or other misconduct. b. The coach/assistant coach/volunteer may terminate their involvement by providing reasonable notice to the organization/league.
Waiver of Liability: a. The coach/assistant coach/volunteer acknowledges that basketball inherently involves certain risks and agrees to release and hold harmless the organization/league, its officers, and representatives from any liability for injuries or damages that may occur during practices, games, camps, or other team activities.
By accepting these terms and conditions, the coach/assistant coach/volunteer confirms their understanding and agreement to abide by them throughout their involvement in the youth basketball program.
