Informed Consent and Acknowledgement

 

Westkhoops Club League Play and Youth Athlete Registration

Please read these Terms and Conditions carefully before participating in Westkhoops Club League Play or Registering a Youth Athlete. By participating or registering, you agree to be bound by these Terms and Conditions.

Eligibility: a. Westkhoops Club League Play is open to all youth basketball players in grades 4-10. b. Youth athlete registration is available for players between the ages of 9-15.

Skill Levels: a. Westkhoops Club League Play welcomes players of all skill levels, ranging from beginners to advanced. b. Teams will be formed based on players' skill level and experience.

League Schedule: a. The Fall league schedule will run from the end of September until mid-November (specific Start & End Dates will be finalized and announced after the League Registration Deadline and based on gym avaliability dates, with the goal of finishing the club season before the start of the SD23 school basketball season. b. There may be a slight overlap in club and school schedules during the final two weeks of league play, allowing players to participate in both the Westkhoops Club League and the SD23 school basketball season at the same time if necessary. c. Each team will have one practice and one league game per week for a duration of 8 to 9 weeks, including a playoff made up of a semi-final, and final championship game. d. Practices and games will take place on the Westside during weekdays at various local gym facilities, including public and private schools within SD23, as well as community and church gyms when available.

Coaches: a. The league is managed by skilled and dedicated volunteer coaches who have undergone vulnerability sector and criminal record checks. b. These coaches, who are often parents and members of our local community, possess significant experience and will offer players valuable instruction, positive guidance, and feedback during both practices and games. c. The individual in this role will have the responsibility of effectively managing and leading their team while adhering to the policies, rules, and best practices set forth by the Westkhoops league and its officials. b. It will be their duty to create a safe, inclusive, and positive environment for learning and instruction, promoting fairness and teamwork, respect for others, and cooperation amongst all parties involved throughout the entirety of the season.

Skill Development: a. In addition to games, skill-building drills will be provided to help players improve their individual skills, including shooting, ball-handling, rebounding, and defensive techniques.

Game Jersey & Basketball: a. Participants will be provided with a reversible game jersey. b. Please bring your own basketball to team practices and games with your name clearly marked on it, for easy identification. Younger age groups will use a Size 6 - 28.5" basketball, while older age groups will use a Size 7 - 29.5" basketball.

Informed Consent and Acknowledgement: a. By participating in Westkhoops Club League Play or registering a youth athlete, you assume all risks and hazards incidental to the activities. b. You release, absolve, and hold harmless West K Hoops, its officers, agents, and representatives from any liability for injuries arising from traveling to, participating in, or returning from selected camp/program sessions. c. You acknowledge the inherent risk of injury in sports activities, including basketball, and waive all claims against West K Hoops and its affiliates, coaches, participants, sponsoring agencies, advertisers, and owners/lessors of premises used for the event. d. You give approval for your child's participation and agree that West K Hoops and its staff will not be held responsible for injuries.

Media: a. Under the services of West K Hoops, pictures and videos may be taken for advertisement and promotion purposes.

Refund Policy: a. Participants agree to Westkhoops' refund policy. b. Full refunds can be granted if a refund request is made at least 10 days before the starting date of a camp or program. c. Cancellations made nine days or less before the start date may be eligible for a 50% refund. d. Once a camp or program session has begun and the participant has attended or not attended, no refunds will be issued. e. The remaining 50% of the payment is non-refundable, covering administration fees, the loss of holding a spot, and coaching staff expenses. f. Medical absence or injury may be eligible for refunds or credit upon presentation of a physician's note. g. No refunds will be given in the event of an "Act of God" or a crisis affecting the global, national, or local level. h. A $40.00 fee will be charged for returned/bounced checks given to the organization.

Dismissal and Exclusion: a. The camp directors reserve the right to dismiss and exclude anyone from the camp/programs for improper attitude, action, disturbance, or disobedience to camp rules. b. If a participant is dismissed from the camp/program, there will be no refund.

Receipts: a. A copy of payment receipts for programs and camps will be provided automatically after the online checkout and registration process. b. Additional copies of payment receipts are available upon request via email or phone call in case of electronic error.

Parent/Guardian Agreement: a. Parents/guardians agree to the drop-off and pick-up times and rules of the School or Gym Facility during camps/programs. b. Parents/guardians are asked to leave the premises once the camps/programs begin after dropping off their player(s). c. Parents/guardians must respect the pick-up and entry times of the camps/programs. d. Failure to comply with the rules may result in player(s) being dismissed from the camp/program without a refund. e. Exceptions to these rules require written agreement between the camp/program directors/coaching staff and the parents/guardians.

Medical Release and Authorization: a. By registering a youth athlete, you authorize qualified and licensed medical professionals to diagnose and treat the minor child in the event of a medical emergency. b. Permission is granted for necessary medical or minor surgical treatment, x-ray examination, and immunizations. c. In case of an emergency, every attempt will be made to contact the parent/guardian.

Assumption of Risk: a. You agree that if you engage in any physical exercise or activity or use any facilities or equipment, you do so at your own risk. b. You assume the risk of any injury, illness, damage, or loss that might result from participating in any activity, including but not limited to basketball camp/program, basketball training, or any sporting or recreational endeavor.

Governing Law: a. These Terms and Conditions will be governed and interpreted in accordance with the laws of British Columbia, Canada without considering its conflict of law principles.

By participating in Westkhoops Club League Play or Registering a Youth Athlete, you acknowledge that you have read, understood, and agreed to these Terms and Conditions.
